Here's how to get your rebate:
- Contact a medical equipment provider to fill your prescription for a ResMed positive airway pressure device (a CPAP, APAP, or bilevel device) and ResMed mask.
- Print out this rebate form.
- Mail the original form, along with the original receipt from your medical equipment provider, to:

Phoenix, AZ 85060-5980 - The receipt from your medical equipment provider must show the specific ResMed device and mask you received, the amount you paid, and the name of the medical equipment provider. (Your insurance company may have to process your claim before your medical equipment provider can bill you and give you a receipt.)
Eligibility Requirements:
Rebates cannot be used for any prescriptions that are:
- Covered under a federal healthcare program such as Medicare, Medicaid, TriCare, or VA;
- Covered under state funded healthcare programs; or
- Covered by an insurer or other third party payer in Massachusetts
Rebates are not valid if your private insurance plan covers the entire cost of your prescription product. This rebate may only be used to cover your actual out-of-pocket expenses and may not exceed $50.00. Offer void where prohibited by law, taxed or restricted. Offer good only in the U.S.A.
Additional Important Information:
- Rebate offer expires on the expiration date printed on the offer. Submitted rebate must be postmarked by the expiration date.
- Submitted rebate must be received within 60 days of the date on the receipt.
- Receipt must be dated after 6/15/2010.
- Submission must contain an original rebate form. Reproductions will not be accepted. Limit one rebate per person. Not valid with any other offer.
